I started on in the blue channel (thanks Ed) and make it a grayscale. I then returned it back to RGB where I adjusted the levels and curves. I used the healing brush (new in photoshop 7) to remove the remaining handwriting on the image. After using the dust and scratches filter I did a little stamp tooling, but that’s pretty much it. The last step I took was to make this a duotone with Pantone 478 CVC.

Very nice work on a tough challenge. I think the tonal values look real good. The only problem areas I see are the baby's forehead, and the area around the boy's face. But it was a tough one to do. I doubt I could do better.

I agree with Ed on the couple points he mentioned and I also agree that you did a great job and brought out alot of detail in the image. Good tone as well.
