Ok, I'm another first timer to this site.... Always up for a challenge I started my first of many..
Well, first I made a couple of duplicate layers and took the bottom layer and applied a exagerated dust and scratch removal to keep the tones of the background after I touched up a few areas I applied a grain to this background using add noise.
Next, I took the top layer and erased the background to allow the bottom layer to show through this kept most of the details of the hair edges. Now the fun began, I cloned out the bad cracks using the clone tool at 100% not worrying too much about the stepping that resulted, then I came back with all the harsh cracks gone and cloned using 30-60% opacity to smooth out the steps and blending better not worring about the blurring at this time after I was happy with the cloning I did an unsharp mask to add some details to the eyes and mouth areas mostly then I applied noise to this layer to give it this grain look and to eliminate the blurred look.
NOTES:
I always try my best to keep the original shapes of an image to keep it true to form - it is easy to get wrapped up in making it look better then the origially was but especially with people I alway try to keep this true look...
I use the "add grain" a lot to cover up blurred cloning and it works very well and eliminates the stepping usually assoiated with cloneing large area.

Great job, especially in the difficult neck area. The hours were well spent since your result is very clean and realistic.

It shows that you have handled the work with a deft touch and a sensitive hand.
The enlarged version could do with being 800px wide, it would also help show off your work to it's best effect.
I like the tonal qualities and I like the way you have handled the hair, it has retained a realistic quality that is all important in the final product.

Now that I can actually view your image (after Gallery glitch was fixed somehow), I can see what a good job you did!
