I straightened the image, changed it to grayscale, then played with curves to get somewhere close to what I wanted as far as lightness and contrast. Then I used clone and push and smudge brushes, sometimes with a texture, to clean up all the damage.
Her eyes were a bit of a problem, so I took a selection of her eyes on a darker layer, and pasted them into the lighter layer, and then just cleaned them a little.
I also tried to fix the background, because sometimes the setting has so much meaning to the people in the photo as well. I was surprised to find there was a man walking by in the background. I hadn't noticed it at first in the original.
