These are the steps I used, though it wasn't nearly as cut-and-dried as it sounds; I still have to do a lot of experimenting before I land on something that seems to work. I may have left out a few things here.

Added levels adjustment layer to remove slight blue cast

Cloned to remove scanning scratch and some blotches on hair and skin

Cloned and brushed to turn dalmatian PJs into plain white shirt (they were cute, but not something I think would be chosen for a portrait) and to add color and texture back into burnt out areas of blanket

Added Overlay adjustment layer to add color back into too-bright areas in hair; added Screen adj. layer to lighten the ear; flattened.

Used Quickmask mode, pen tool and brushes to isolate and mask out background/sofa

Duplicated layer and used airbrush and Gaussian blur at radius of about 2.3 to soften edges and even out skin tone; added a layer masked and brushed back in details in eyes, lips, hair, dog's fur, etc. Also brushed lightly over freckle areas to reveal them a little (my daughter has freckles too!)

Accentuated eyes by dodging over irises and adding some white highlights and black fringes.

Created new background with airbrush and Motion blur filter.

I think your description is an example to follow. Real nice effort in detail and the image turned out great. Keep up the nice work.
