Hmmm -- this is the hardest part for me, remembering what steps I used. Basically:
Removed many spots and cracks by duplicating the layer, running Noise/Median, then adding noise and blurring that noise to simulate the picture's existing grain. Layer mask to paint only where I wanted to remove the spots.
Clone and healing brush on remaining cracks.
Replaced part of the curtain on the right side.
Softened some of the highlights on her face using brush in color mode.
Replaced bottom part of the dress to match the rest.
Levels and color correction.
Blurred background, leaving SweetP and Mr. SweetP sharper.
Still don't know how to remove the grain without it looking "doctored."

Scott you have maintained consistency in the color of the image. The very troublesome bottom in yours is seemless. Her eyes seem a bit fake --- but I know there was not much to work with! I am still working on mine!
