Used Flora's tutorial "Enhancing details in unevenly faded pictures"(channel mask) with 2 or 3 multiply layers and plenty of mask painting. Cleaned up with clone. Copied and fiddled with left hand to add a right hand and arm. Smoothed skin with guassian blur and layer mask (Vikki's Awesome power of layer masks tutorial).
Very nice, I like how much detail you were able to recover!

This result is very impressive. The details are well toned and have a nice texture (not too smooshed.) The background has retained much of its charm and aged look. The subtle changes to the left hand are wonderful and add to the realism. You obviously have a knack for knowing when "enough is enough!" Bravo!
