I made several efforts of which the first one with gblur didn't look real because of the ghosting you don't seem to have with the lens blur in CS (I have ps 7).

For this one I used the following method :

Copied bg layer. On this layer I started the blurring:
I tried kpt equalizer for getting a blur without the ghosting part.
I found that if you blur the pic with small amounts for the 1 and 2 pixel settings you almost have no ghosting effect (the blur is not substantial though.). I ran this kpt blur 4 x in a row on a background copy layer.

Made a second copy of the bg layer and put on top.
Sharpening up in kpt equalizer

Then I used a layer mask to make different parts of the image look blurred in a different way.
I wanted to make the closest image part in focus : i.e. the hand with the candle holder.
I made a gradient on the mask over the surface of the table to the end to blur that part gradually.
Painted the curtain in the mask all black and the hand with the candlestick totally white. Then painted in the parts of the foremost figurine depending on their distance to the so called "lens".
Masked little figurine in the middle too relative to the "distance"
