This looked like fun until I got into it a ways. I desaturated the sceen capture and cut out each face segment and placed them on separate layers. I used transform to adjust each to approximately match proportions and moved them into position.
At this point, I decided to make a "before" image, which you see on the left side, for comparison.
I filled in the gaps using mainly clone on many separate layers and adjusted opacity and dodge/burn to match adjacent areas. Used smudge to blend some troubling transitions. Flattened and added some noise (too much but I didn't notice until after I had saved it -- too late!). Added a little blur in an attempt to compensate but I'm not happy with it.
Erased the neck areas and made it a floating head. Kind of reminds me a little of a pencil sketch.
