Clone Stamped all blemishes and the tear
Increased RGB levels in the grey area to the right of the mouth
Moved the right side of the mouth left slightly to compensate for the tear
More stamping to try to smooth out difficult areas
Increased Brightness and Contrast by 8

Very nice clean-up and repair. Leaving the old print texture and sepia kept the antique look.

The repairs of the face are very well done. I think repairing some of the lesser damage to the rest of the picture would have helped the face stand out more and be the center of attention. It might just be my eye, but it insists on travelling directly to the damaged area at the bottom left corner!
