I first of all selected the background with magnetic lasso used dust and scratch and a high level to smooth it out. Used brush and quick mask to select skin on face and neck and again employed dust and scratch filter but at a lower level to "de-wrinkle" the face. Used the history brush to bring back a few details lost plus a lot of cloning.

Very nice job on the repairs. It does look as if some of the detail from the right side of the cheek is lacking or too smoothed out. But a nice looking portrait in all!
