Using Photoshop CS, I made a copy of the background layer.
I then used the Clone Stamp & Healing & Patch tools to carefully remove all the damage.
Copied right eye onto a new layer, moved this over to the left eye, then using a layer mask
bought back the left eyes original eye lids.
Mouth was again cloned, when cloning I set the opacity & flow low.
I created a new layer setting it to overlay, then using the high pass filter to create a sharper image.
A layer mask was applied to this layer so that it would only reveal the eyes etc.
New layer was made for background.
Applied layer mask then using a soft brush setting removed the original damaged background.
Finally, used burn tool to enhance shadows and detail.
I wanted to keep this looking as close to the original as possible

Nice job, and good description too.
