Well, I've been lurking for a while now so I figured I'd finally post something. :P
1) Cropped slightly to remove damaged edges
2) Duplicated background. Changed duplicated layer to grayscale (lighten mode, about 10% opacity).
3) Used the clone stamp tool, healing brush, and patch tool for scratches.
4) Used the pattern stamp tool to restore some of the uneven textures from cloning
5) Selected the his right arm, duplicated it, moved it overtop of his left arm and burned in the shadows. Same thing for his ear.

Beautiful restoration. Only thing I can see that would add to this would be a bit of darkening around his head to bring him out of the background a bit more.

I think this is a beautiful restoration, but the symmetry of the arms is a bit of a giveaway.

Apart from the symmetrical armpit (I'm not saying I would have noticed if it wasn't mentioned), I think this pic best matches the tone and feel of the original - looks like an old photo in excellent condition. I especially like how you handled the bg - leaving it un-homogenized.
