with Photoshop 7:

Cloned out the cracks and discolorations and smoothed out the angles usind smudge tool.
Hair, eyes, lips, dress, and skintone all done on seperate layers.
Had a very hard time finding hair and eyes to use because of the angle this photo was taken. For the hair I dragged it in and sized it then erased around the edges until enough of her face was showing. Then I copied some from the left side onto the right side. For the eyes I did the same, erasing all the "extra" skin leaving only the eyelid creases and the bottom eyelashes around the eye-the rest of the eye area is the staue. I did hue/saturation on the eyes and hair then merged visible.
For the lips I painted in color then added texture from a close-up portrait I found. I did hue/sat. on that layer as well plus added noise: Gaussian and Monochrome at 5%.
Finally for the skintone I selcted my colors from the same image I got the lips from, did them on their own layer at 100% opacity so that when I was done painting I had this big multi-toned pink mess. Then I ran gaussian blur on that at about 3,changed the layer opacity to overlay and dropped it down to about 75% opacity.

That's about it, except for a few little touch-ups where some of the layers didn't blend together all the way......
