Used the blue channel as the basis for a B&W image. Ran neat image and polaroid dust & scratches, then used layer mask (hide all) and painted in corrections. Flipped image to provide left jaw/right sleeve. Lots of cloning. Bit of gaussian blur. Rendered a new cloudy background and finished off with a vignette

Darren, nice neat job on the skin, although I can still see where the crease was.
