First desaturated
Removed the larger scratches with Image Doctor Scratchremover and smaller marks with Spot Remover.
Used Healing brush and blur a lot.
Added Levels adjustment layer to lighten and reveal edges.
Tidied up the signature cut it to another layer to be able to clean around it easier.
Flattened.
Added grain to take away the false look.

A very nice job Peter.
I like that sort of mysterious misty look you have achieved.
Great facial details, eyes, mouth, and nose.
Just one minor thing that my eyes are drawn to. - The left side of Lori's neck, looking at her, - seems a bit thick.
It really is only a minor thing!

You may be right about the neck, that comes from working at high magnification maybe.
Thanks for not mentioning the necklace, it could have been better.
