I used the clone stamp tool and cutted and pasted small sections (at varying opacities)to clean up the damage.
Damage to the right ear was extensive, so I borrowed from the left ear to reconstruct it.
I spent a lot of time cleaning the skin areas, and decided that I rather liked the effect of clear skin on the still rumpled clothing and background. To me it cleans and restores the image, but still retains the feeling of "old".
I also converted to greyscale and adjusted the tone, then set back to RBG and used the saturation controls to get a sepia tone I liked.
Finally, I sharpened very lightly to boost contrast, (20, 50, 5), and called it done.
