Using GIMP, I first duplicate the base layer and cut out the small doll
I then create a layer mask as shown here:
I then execute the Gausean Blur filter twice with these settings:
I then flatten all layers. I made the main focus the small doll. I wanted everything behind the small doll to be very blurring, while in front of the small doll just slightly blurry. Just my take.
