I started by adjusting the contrast with Levels and then repaired the damage in the background areas of the picture before using the Healing Brush and the Patch tool before starting on the child's head. Again using the Healing brush, I repaired the black dots on the face and smoothed out the hair. I then created a new layer with blending mode set to Multiply and over painted the hair and face with tones selected from the original. I faded the over painting with the Opacity Slider until I reached a point where, in my opinion, the over painting didn't compromise detail. A slight bit of blurring with the blur tool set to 15% further smoothed the facial area.
I selected the gown and put it on a separate layer set to blending mode Lighten and applied the dust and scratches filter. This cleaned up 80% of the gown. The balance was done with the Healing Brush and the clone tool. Where I found areas of lace detail, I would select them, move them to their own layer, and using Free Transform move them to appropriate areas of the gown. This covered up areas where the repair had obliterated what little detail there was.
I applied a small bit of unsharp masking to sharpen the image and finished up by increasing the canvas size so the oval could be completed.

I was about to abandon this photo ,but now am inspired to try again. Going to try some of your techniques. Not sure what healing brush does but hopefully there is something in PS6 that would be similar. Thanks for working on photo.Joyce R

JB,
Very nice work. You've done a fine job on both the face and the dress. The tonal values look near perfect. I don't give too many 5's, but you got one.

Great job with the tonal values and with making the dress an interesting element again. I think maybe some upper areas of the dress look too much like smudges though and could be whitened out.

Thanks for the kind comments.
Tom,
I agree that the upper area of the dress looks smudged. I thought if I tried to repair it, I would do more harm than good.

Good repair job, and nice tonality. I notice you eliminated the person in the bg. I have no problem with that, but perhaps that leaves the bg itself a bit dark?
