Using PhotoImpact Version 5.
I did not find that great white expanse very inspiring but the owner of the photo did so I decided to give it a try.
De-saturated then applied Tone Map settings of Hilight: -15, Mid: +25 and Shadow -25 which gave the photo better tone and helped emphasize the flaws.
Working on the face and hands: Clone tool fixed the forehead and Dodging and Burning repaired the cheeks (a real pixel party). To de-emphasize the too-prominent texture in the skin, the face (not lips, eyes, nostrils) and hands were Selected, converted to an Object (layer) to which I applied a Gaussian blur (of 5). To this blur I added Varied Noise (to match the grain in the rest of the photo) at level 15 then changed the transparency of the object to 60%. This object was then merged (flattened?) with the base image.
Working on the gown: The smudges were Dodged away. There is a "hot" spot in the upper left quadrant of the gown which I filled in by Burning in some new folds. The existing folds were emphasized a little by Dodging the highlights and Burning the shadows a bit. This gave the gown more depth. Remove Scratches tool took away the easier stuff then more Clone, Dodge and Burn for the remaining. I strained the Sharpen tool to get the detail in the lace.
I selected the whole gown and applied the same technique as to the face to de-emphasize the unwanted blotches.
The remainder of the image was cleaned up some then sharpened (including the baby's eyes). A touch of sepia for warmth and "authenticity" and then a click of contrast for presence.

Excellent repair job. Good sense of subtlety. The dress seems a bit redder than the rest of the image, and perhaps a bit dark.
