I used a lot of techniques from Katrin Eisman's book for this one. I removed the color using a chanels adjustment layer. To get rid of some of the dust and scratches on the background, I selected the right and left background areas individualy abd moved it to a new layer, nudging it down and over 2 pixels then set the blending mode to lighten. I used the clone tool to cover any white scratches. I used the healing brush on the rest of the flaws and dust. To use the healing brush I used this technique from Katrin's book:
I selected an area in the lower left hand area that I thought had a pretty even texture and shading, an area that was not too smooth. Using the pattern maker filter, I saved the seection as a pattern. When I used the healing brush I selected the source as being this pattern intead of sampled. I healed the entire photo this way. Looks good without being too smooth. The right eye I fixed by selecting the left eyeball and pasting it over the right eye on a new layer, erasing so it fit inside the eyelids naturally. Used dodge and burn to recreate the creases on eyelids. For the damaged ear, I copied the good ear onto a new layer and used free transform to get the angle right. Lots of time was spent with the healing brush on this one.

You've done a really good job on this. I didn't think of using the pattern maker filter sounds very interesting. I'll give it a try on my next restoration.
