My first post! Worked a really long time on this; gave up on it, thought of new approaches, started over many times. Tryed a lot of things that didn't work and deleted a lot of layers along the way.
I started with about 8 layers of the decrack action found on this website, mostly for white cracks and a couple for dark ones. Merged visible. All this created a lot of dark blotches.
Used high pass to select dark blotches and created a levels adjustment layer to tone them down.
Did a lot of spot healing, starting new layers as work got finer. Started looking too light, needed to adjust overall levels back to original darkness.
Eyes were losing definition, so I copied and pasted the original eyes back over everything. Cloned and spot healed to bring back brightness.
Did a lot more decracking on hair, plus former blotch-removal trick with high pass and levels.
Patched large holes in image with copy/paste. Used combo of regular and pattern healing brushes to smooth background.
Cropped image slightly to have better symmetry.
Did more decracking and spot healing on dress, also some clone stamping.
Painted in details on blank layer with low opacity brush (such as eyebrows).
Used motion blur to recreate hair texture in 3 directions/layers with layer masking to specify direction.
Clone stamp/low opacity paintbrush on new layers to correct final details/flaws.
Sharpened here and there, then added noise.

Excellent job preserving the shape of her face and facial details - 'likeness' is right on. Just needs some smoothing of lines and shadows.
