Only after I got the basics of the retouch over with did I discover the hidden difficulties. Although not 100% solved, I'm posting anyway.
Technique: Replaced blue channel where most of the noise was and added adjustments to bring back the depth lost in the replacement. From there...lots of blurring, cloning, masking, some dodge and burn, and levels adjustments with more masking on those levels adjustment layers.
Jpeg compression took its toll and some of the parts that looked good even at 100% size look a bit splotchy now at the smaller size. Go figure.

That looks very nice, Janet. Face and hair have good detail and her skin is de-spotted without going overly smooth.

Fine work Janet,
Good work on the skin. It was tough.
Janet - You squeezed your jpg down to 93kb.
We can go to 200k. No wonder it looked a little splotchy.
