I first used a channel mixer adjustment layer to make it black & white.
Then I ran an FFT filter to smooth out all the texture, then another curves adjustment layer.
I cloned all the spots and used a duplicate and move technique. One layer was lighten and one darken with masks to blot out the spots.
I used the pattern stamp on the background and the blouse to keep some of the texture.
More dodging and burning....
Then I colorized it painting on blank layers set to color mode using various hair brushes set to clone or brush.
Then I used a blend if layer option for the hair and the blouse so they would blend.
More colorizing and fine tuning with dodge and burn tools.
That's about it... I forgot to write down what I was doing, but this is pretty close.
