Definitely a challenge for me because I'm pretty new to this.
Loads or clone work for the face and a little work with the healing brush (I didn't find it very succesful for this picture) and copying of sections for the dress. Clone tool for the background and the hair is a mixture of a little clone and some hand paint and minor highlights. Treated the whole thing to a minor gausian blur and used the burn tool to darken the eyes. Then I cropped it down and made a mask layer for the dress which I used to increase contrast and lower the brightness.
