I began by creating a copy of the original image and converting it to grayscale using the Channel mixer to base the grayscale version off of the green channel. Then I created layer filled with neutral gray above the grayscale image and set the blend mode of the layer to overlay. I then used the dodge and burn method on this overlay layer to even out the brightness around the cracks. After doing this I merged the dodge and burn layer onto the grayscale layer.
I then created three copies of that layer and ran each one through the "Dust & Scratches" filter with increasing strength each time. I added a layer mask to each of these three layers and filled the layer masks with black so that the layers were invisible.
Then I used the brush tool to paint on the layer masks with white and make only selective parts of the dust & scratches filter visible. For small cracks I paint on the layer mask of the layer that received the weakest dust and scratches filter and for deep cracks I use the layer that went through the strongest application of the dust and scratches filter.
I continue this process until only minimal cracks and scratches remain, and those I remove with the clone stamp tool. After I've finished repairing the damage to the grayscale version, I added back in the original colors by creating a copy of the original color image on a layer above the restored grayscale image and setting the blend mode of that layer to "color." Then I used the free transform tool to straighten out the image and cropped it.

I really enjoy seeing what a good job you did with this picture. I've been working on it using the clone stamp tool and spot healing tools. Following your description, I think I will go back and try to make some improvements.

Wow, I'm impressed! The color looks very authentic, although a bit dark to my eyes. I want to see if I can walk through your directions and see if I can reproduce your results. If I can, I will have learned a lot. Thanks.
