In Abobe Photoshop CS3 Extended:
I used the Spot Healing Brush to remove freckles.
I used the Sponge Tool for desaturating some red spots.
I used the Dodge Tool for lightening dark areas like beneath the eyes and harsh neck shadows.
To whiten the teeth I sloppily circled them with the Lasso Tool in a duplicated layer. Then I went to Selective Color > Yellows and moved the slider for Black to the left. Deslected that. Added a mask and erased everything that wasn't the teeth. Merged the layer down. This is also how I whitened the eyes, except I moved Selective Color's Neutrals slider, not the Yellows one.
To make the woman thinner, in a duplicated layer I used the Liquify Tool > Pucker and Forward Warp. Then I applied a mask and tweaked some areas back to normal, like the baby and her facial features. Merge down.
