Duplicate

Layer > New > Adjustment Layer > Levels … use the Levels Eye Droppers to determine White and Black.

Select Baby and put in own Layer > New > Layer Via Copy. Filter > Noise > Dust and Scratches, Radius - 6 and Threshold - 25.

Used Clone Tool where necessary.

Filter > Sharpen > Unsharp Mask, Amount - 73, Radius - 2.3 and Threshold - 14.

Made a new background using Linear Gradient Tool … going from gray foreground to back background.

Filter >Artistic > Sponge, Brush Size - 2, Definition - 0 and Smoothness - 5.

Moved Boy to Background.

Flattened.

You've fixed some of the problems with the baby, but the facial skin now needs smoothing. I agree with Ken about the "floating effect" -- you've cleaned up the background scratches, but lost the shadow of the seat/cushion on which the baby is seated, so now it looks like a floating disc of light. Happens to me, too, so just keep working on it.

The original background is so bad it's tempting to throw it out. It's harder to create the replacement though. There's probably still some more repair work on the kid, especially the face.

I applaud your efforts on this difficult baby. The background needs a bit of work to feel more like it's part of the photo of the baby but I think you did good.

I think the baby looks pretty good. I tried getting rid of the background too, but it just didn't look right, didn't look natural. I think when retouching something so old...it's okay to leave it looking a little rough (just my opinion).
