Started with the healing and cloning tools to fix scan marks and blemishes. Used the clone tool to remove the background as well.
Varied the hue and saturation to change the 'warmth' of the skin tones and duplicated the layer.
On the copied layer, gaussian blur was used and faded for softer effect - and her eyes were erased on 1st layer to make them appear sharper.
Cropped in tight to accentuate eyes and composition

I really like the way you've cropped it. It draws attention to the girl, and it works excellent.
