My goal was to keep this as close to the original photo as possible. This took me about 90 minutes... the biggest challenge for me was the noise and artifacts in the skin. After fixing levels, I got rid of the dark noise first by duplicating the layer, running median on the dup, changing it to lighten mode, and moving up and over a few pixels. Then I masked so only the skin is fixed. That left some garbage on eyes, mouth, hair. Which I fixed mostly by smudging. Lastly I added a vignette background to replace most of the back original, and the original, blurred, overlay in color mode to bring back the original sepia.

This is excellent,really nicely balanced and natural looking.
I'm not sure what you mean about moving up and over a few pixels?

Thanks Spotter... on the duplicate layer which is on top of the original, I changed it to lighten mode, then with the move tool selected, I nudged the top layer by arrow up, arrow up, arrow right, arrow right. Come to think of it, since I had blurred to the top layer I probably didn't need to do that.

Oh I see,physically moving the image!
I have been having a bash at this one and find it quite difficult with all the fizz and blockiness .
Yours has very nice smooth skin tone.

My biggest challenge was removing the garbage in the skin without making the skin look blurry. Not sure I entirely succeeded... An important part of my technique was the smudging which helped eliminate the noise in skin and details like the around the eyes and hair.
