1. Open original skull image in Photoshop
2. Adjust levels
3. Extract skull onto a separate layer
4. Paste in face of family friend with strong wrinkles
5. Scale face to size of skull
6. Clone out the eyes; erase mouth area
7. Set face layer blending mode to multiply
8. Make extracted skull layer & face layer a clipping group
9. Have a beer
10. Clone in worms
11. Add texture with Lighting Effects
12. Add mood with levels adjustment
13. Tweak image with manual dodges and burns

I will have to remember step #9 if I ever get a client who wants worms added to his pic.

As disgusting as this is, in the few milliseconds I actually allowed myself to look at it, I couldn't help but appreciate the skill and creativity you put into this monstrosity. Extremely well done.

Frankly, I can't even look at it in large view. The Thumbnail looks brilliantly creepy and from reading remarks above, by braver souls than I, this is another arrestingly good image from visual eyes. I leave now, without looking!! Art can just be too powerful for me sometimes...bawk,bawk,bawk....
