How do you fix teeth problems such as chipped teeth and gaps?

Here is a great example

In this case, I made a couple of patches by copying and pasting each of the two front teeth to a separate layer (one to each).

Then I moved each toward the other. This made the teeth slightly wider, but it's not too noticeable with this picture.

Finally I cloned out any "joins".

Obviously with different problems you'd have to employ different techniques.

This was interesting. Gave it a try. First, I enlarged the picture in Blow-up and then selected the tooth with the pen tool and did a transform to pull the tooth towards the center of the gap. Did some painting to fill in the areas.

My try - I selected the right half of the teeth at the boundary of the teeth/gums and lips. Copied to a new layer and used free-transform to "stretch" all the teeth by moving the handle nearest to the center tooth. Then did the same to the left side. This makes all the teeth larger by a very slight amount and is not as noticeable as only widening the center teeth.

I tested another way, I cut/paste the left/right sides of the teeth on separate layers and moved each three pixels towards the center.
