Maybe is too easy for some of you but I have no idea... Please, give me your advice in how to take out this stains while keeping the image integrity. I also want to know your ideas to correct the tilted change of tonality across the entire photo...

Hard to give advice without the photo... but never stopped me before..lol

Duplicate your background.

Click on the top duplicate and desaturate...ctrl+Shift+U... if the red disappears.. then you are in luck


Delete your temp duplicate

OPen a blank layer

Set the layer mode to Color

Sample good tone next to the red stain and paint over it

Just a quick way there are others.

The stain cannot be removed using my original technique… but there are always many ways.

I duplicated the background then used apply image and applied the red channel at 80 percent.

Desaturated this layer then just used the healing brush and clone to blend and remove the damage.

Then a levels adjustment to darken slightly

I used noiseware to reduce the paper texture.

It would be easy to add back tone if desired.

And your explanation is so clear, this will be a very good practice for me... Bad thing I don't have that healing tool, it seems it came since PS CS2. Neither have Noiseware, so it could be a litle harder, I don't know.

Here is my try at this.

Well I first looked at the channels and then selected the red channel 100%. That eliminated most of the problem.

Next used the healing brush

Finally used neat image to remove texture

philbach (and everybody on the thread), thanks for the tips, looks smooth and clean. I have only a concern but happily it brought something important to my attention: that weird little eye in the fatter twin (the one at left). That made me check again the original and the other versions and as a matter of fact you can see the same problem but less noticeable. Finally I was able to return to the original paper copy and discover the problem: the photo is broken (sorry for my limited English I know is not the perfect word), ok, broken just through this eye. Actually, the red stain follows the broken (or whatever) path. The photo was just put together with tape fairly well. Now, the good thing is, it adds a new challenge to the repair: to fix the eye. By the way, I know this guys and are perfectly normal, not weird eyes at all. Don't know about a temporary medical condition (i.e. a punched eye). That eye have problems in size, height, orientation and brightness, I think. Sounds like an interesting study on the human face. Stare first on the other twin and then on this and you'll sense something is wrong with his sight. Please, give your opinion.

Well I looked at your original and saw that the cut does go through the inner side of that persons right eye, but I don't think it distorted the eye. Enclosed is the original picture just using the red channel. So I think that when the picture was taken his right eyelid was drooping some for whatever reason.

Yes, philbach, maybe is just that, the sun in his face from that side. I was thinking about a past thread about correcting the orientation of somebody's eyes (not this case but maybe interesting to see what they did), I'll make a search... Thanks.
