As a start...

I adjusted the levels for each channel individually.
Then ran NeatImage (neatimage.com) on each channel individually.
Used Channel Mixer to get to grayscale.
Used techniques from Flora's tutorial here (http://retouchpro.com/tutorials/?m=show&id=65) to enhance detail.
Used healing brush to repair some of the damage.

Obviously you could take this further.

Level Command
Dust and scratches
Neutral Grey layer to bring out shadows and highlights more

and DESTURATE command to make it B/W

play with it :)

just a thought.

In this case, I used Noise Ninja with filtration set to 200 IS0 Nikon Coolpix 5700.
Levels adjustment layer. Balanced R,G and B.
Adjusted tones with Shadow/Highlight filter in PS CS. Using Curves, Channel Mixer and other tools can also be effective.
Ran image thru High Pass filter set to 11 pixels. Used Fade Back set to Overlay.

Sorry for the delay, but here is the detailed description of what I did with your picture:


Duplicated the Background Layer.
Used USM (Amount>20%, Radius>50, Threshold>0) to enhance the contrast.
Merged Visible. (here (http://www.retouchpro.com/forums/showpost.php?p=67894&postcount=18) is how to do it without losing the underlying Layers)
To bring out the faded details, I used 'Method2' as described in my Tutorial (http://retouchpro.com/tutorials/?m=show&id=65), duplicated the luminosity Mask Layer a couple of times, decreased its saturaton and changed the blending of the last duplicate to Soft Light.
Merged Visible, and, keeping visible background and Top Layers only, I created a (white=reveal all) Layer Mask and, with a black soft brush, I painted over the parts of the image I wanted to restore to the original.

Working on the merged Layer, I run Polaroid Dust & Scratch Removal Filter, Dark Dust first. The Filter can be downloaded free here (http://www.polaroid.com/service/software/poladsr/poladsr.html).

Added a Layer Mask (Black=Hide All) to the D&S Layer and with a soft white brush I painted on the Mask to show the corrections.
Merged Visible.
Repeated the Polaroid Dust & Scratch Removal + Layer Mask, for Light Dust on the newly merged Layer.
Merged Visible.
Used Neat Image (softer settings) to minimize the noise on the last merged Layer. (Neat Image can be downloaded free here (http://www.neatimage.com/download.html) ).
Used the Patch Tool to fix scratches and spots.
Created Several new empty Layers, Blendings: Lighten, Darken, Overlay and Normal, to correct, emphasize selected parts of the picture. (Lighten the still darker spots, darken too light areas, add dimension, correct discolorations, etc.)... concentrating particularly on his eyes (pupils, catchlights etc.).
Merged Visible.
Used Curves Adjustment Layer to enhance contrast and tone.
Merged Visible.
Used USM to slightly sharpen the picture.
Used Hue&Saturation Adjustment Layer (Colorize box checked) for a light sepia tint.
