Please Give Critique of Retouched Portrait

Greetings all!

I am an intermediate Photoshop user, and I could really use your opinion on a challenging retouch project that I'm doing pro bono for a friend.

The original and retouched portrait are shown side by side here:

In particular, I could really use some advice on how to make his smile appear more natural. As you can see, I replaced the subject's teeth and altered their lips. To make the teeth look more natural, I applied a gradient shadow to the back teeth. But, the back teeth still don't look totally natural.

Thanks for any advice you can offer!

I like what you've done so far, it kind of reminds me of the way they treat some of the Saturday Night Live photos. Two things that I would address: 1) His left arm looks strange and cut off and 2) There is a lot of noise in the jacket that needs reducing.

Here's a quick (messy and perspective off) example of my suggestions. It will take a little fiddling, but you should be able to use his right arm to recreate the left arm.
