I think replacement would be the quickest route, finding another image (if possible) with a wite coat. It can be done with out replacement but will take a while and with the shadow area it will be tricky to make it look convincing.

Really good job blending in the new coat.

Ultra nitpicking mode on: edge of closest glove could do with blurring a little, like in the original (looks slightly cut and pasted), and cuff area of other glove not quite right, but if you don't show them the original, no one would know.

Excellent piece of work though.
