don't know whre to start..

Hi I'm new to this forum and I was wondering if I can get some help to get me in the right direction with this photo my big problem is the stain on the bottom, the lip and teeth and the old texture of this photo, I can take care of the hair and all of the cracks using the clone tool, patch tool, and healing tools. I started with the levels adjustment layer and then I made it black and white I realized this photos is really messed up any help I can get would be really apreciated, thanks. I'm using CS3 and a wacom tablet.

If you convert the image temporarily to LAB color, you will find that the stain is easily selectable in the B channel (see attached). Once selected you can adjust the level to match the tone to its surroundings.

Fred, here are a couple of other suggestions.
For the hair, you can just select the wedge where it is missing, rotate and drag the selection to copy a section of the rest of her hair.
For the teeth / mouth, one option to consider would be to find a set of lips / teeth on a stcok photo site that approximate the subjects. Copy and paste it over yhe mouth area. Do an Image>Adjust>Match Color to match the tone. Add a layer mask and paint around it to blend it in. In the example below (not a good match for this model - only shown as an example) the model was in color with strong red lips. Match color blended it well.

It may be cheating a little but I found that someone else's teeth fit just fine and for the stained dress someone else's white dress masked to the original looked OK... the arms could be cloned from the chest.. and some cloning and healing as you mentioned should take care of the rest

thanks for all the advice guys I will be posting my progress pretty soon I've been working too much but I will have some free time over the weekend, klassylady amazing work...

I just got rid of a lot of the damage by adding a frame.
The adding teeth is a good idea.
