Probably foreground image was blended in with background.
The background was given painterly look. Foreground picture was smoothed and enhanced. Probably some tone mapping was involved. I apologize for these instructions are not very precise, if you want to re-create the look it would be trail and error....

Definitely some tone mapping. Aside from that It's hard for me to see the details in the image to be sure.

If you look at the website, it's run by an artist who hand paints backgrounds for photography studio work. This looks like a studio photograph shot in front of the muslin background he sells as "Golden Hills". It is beautiful.
