Had a go with this a few days back, and then left it sitting in a folder (sorry).

Didn't do too much other than a bit of cloning, a few levels adjustments, then used Neat Image in a couple of masked layers to take out some of the noise.

Added a new Matt as well.

Here's my try using Neat Image and cloning/healing. I generally use Neat Image BEFORE any cloning or healing. The cloning and healing tools remove the noise where they are use. Neat Image assumes that the noise is uniform across the photo.

Quite a subtle one this, as the line between over smoothing and not is quite a fine one. Its easy to lose some of the finer detail (hope I have't done that), still it's very much a judgement call.
