this is almost all clone and airbrush. just takes a bit of careful work. there is also a color balance and a hue/sat or two to adjust some of the colors and saturation. i also doubled the image size to make it easier to work on. oh, and i also ran a 'digital camera noise removal' on everything but the face and collar and neck.

edit: i also removed the lens reflection in his glasses over the one eye.

Have you tried to remove the ink physically?

A piece of cotton and 95% alcohol can do miracles. Just test it on a corner to see if it removes only the pen ink and does not ruin the picture. If it work the job will be much easier.
I read the article here in Rpro and tested it, it worked really good for me

here is mine
i just used patch tool,brush tool and clone stamp only
then add a little contrast and Neat image for noise removal
