Neat Image question for Flora

Flora...

Could you explain how you go about using Neat Image? When you decide to use it and how you use it? Ive seen other posts in which you use it just on a shadow mask... but whenever I seem to think I should use it, it always seems to be on the entire picture and it doesnt always seem to give me the result I think im going to get, so Im not sure Im using it right.
