1) Make a good greyscale image - combine the channels in the channel mixer;
2) Using curves, do an S-shape, lifting the lighter parts of the skin and lowering the darker parts;
3) Apply duo-tone colouring to taste.
A fun thing to do in these cases is to "de-construct" the image (I just did the colours very quickly). As you can see, by un-doing the above steps you arrive back at a "normal" image.
