Here is one way to get smoother skin that doesn't use any method that has been mentioned yet:

I use it all the time, it works well for me.

I've actually used that technique once where there was a lot of blotchiness. Pretty effective but not a lot of control. I'd consider it down and dirty since you're relying on the cleanliness of the Red layer.

I just tried d&b on this picture I'm retouching, using MadClark's method, and it made a huge difference to the appearance. I was about to give up on her skin but this worked wonders. I'm going to do more but here is the result so far:

You're right it does depend a lot on the picture. Sometimes it works really well other times it hardly makes a difference. It's worth a try though, especially because it's so easy

Yeah also at times other channels can be used in place of the red channel as well as using calculations to create the new layer as well basically looking for or making a layer that contains the best looking skin. Many ways to skin a cat in PS....
