help! too many pixels on picture

hi ppl, i really love this pic and ive tried everything i could to retouch this picture but for some reason it remains the same.. can some one help retouch it.. i really do not like the pixels standing out

I used Topaz Adjust noise filter on it, makes it somewhat better.

What have you tried and what program are you using?

Lens correction (which I do not have) is almost always necessary when dealing with arm length photos
