Dupe the Red/Cyan channel. That has the least amount of skin detail, but usually retains the stuff you want to mask out like eyebrows, eyelashes, nostrils and the hairline edge.

Burn the areas to mask. Yes, that's with the burn tool. This is the one place the dodge and burn tools are not only effective, they're more so than most other approaches. Dodge any residual shadows on the skin. Fill in the edges around the cheeks and the eyes with a brush, as if you were painting a quick mask.

For mask creation, dodge and burn tools work well. They darken the darker pixels before the lighter and visa versa. You mostly want to define the hair edges, the lips, the eyes. The rest can be softer.

i need super speed and acuracy, just painting it in seems to be the most efficent for me.

i use a generally soft edge, 100% opacity. in quickmask. all the other stuff requires too much cleanup and finessing. not a time saver in the long run.

just out of interest, i'm just getting starting in retouching people rather than landscapes but what would you use a skin mask for, is it as simple as just making adjustments to the skin and nothing else? thanks

I use a bunch of different techniques depending on the buildup of the image.
Selective color can isolate skin pretty well. A combination of channel blending usually gets it in about three clicks. Calculations can get there with a little brushup after.
There are ways to get a nice mask of skin with a gradient map if you know the luminosity of the skin color.
