Water Hands

I've found this picture today and I was wondering if anyone know how to do this?

Go to retouching->recent work - and try to find a picture where "water hands" are making a massage.
Somewhere else I've found that this hands were made from the normal hands so they were converted... and my questions is how to do this magic?
