I use the split technique you talk about but I just work on things per layer. So first I do the main bits to be healed and then work on the tones layer only. Then d&b.
I found that the Dgrunge technique helps but just using it slightly.

I applyed the Dgrunge, but I just see it balances the luminosity making the image flat looking if going too deep. I use that to soft the transitions between luminosities. But how can I even the stains found in the skin? I guess is the same technique, but I may wrong the blur amount. Mmmhhh... I think a lower amount of blur will take those patches, as you said, the lower the radius, the smoother the details...

No prob. I thought you didn't get the message. By the way, I did some tests with different blur radius with the split technique in order to see the difference we were talking about and here it's how it looks:
