Surreal - how to get this effect

I wonder if "Filter"-"Blur"-"Radial Blur" in Zoom-mode would help. Along with a black and white adjustment layer and a yellowish tint to get the color and a new layer with some dust and scratches on top of everything to make it look a little more interesting.
Oh and the "Filter"-"Noise"-"Add noise" on a new layer filled with 50% gray on "Soft Light"-mode could help as well.

The blurring looks very much like the Radial Blur filter in Photoshop.

copy image, merge major offset left softlight mode
copy image, merge minor offset left softlight mode
zoom blur to subject
add noise or overlay fog
convert to shades of gray or black\black-white gradient
adjust as needed

I tried a few images using this and they are pretty close to the example. Probably six of one and a half dozen of the other on methods though.

I tried the zoom lens first, but my camera wouldn't let me click and zoom at the same time. A manual zoom lens would work though I bet. Next I tried setting up the shot and as I clicked the shutter I pulled the camera back. The image had many of the features of the OP's example. Double features and blur. Neat little trick, thanks.

Then in PS you can apply Filter/Radial Blur set to zoom. You will figure out the other stuff.
