changing or creating light direction

if you have a very flatly lit photgraph, is it possible to make it look dramatically lit from behind? or change it from a flat to a strong brightly lit directional light, say from the left side casting shadows along the right side of the face etc. is it possible to do this and not look fake? anyone have a before/after example of that?

It is possible and can be done, but you need a lot of knowledge of the shape, surface etc. of the object you want to change the lightening on.
For example you could use a new layer on "Soft Light" blending mode and darken particular areas, lighten the other side and bring in some shadows.
I have actually no example of my own to show you, but Quantum has done something similar in his portfolio:
