How do you achieve this skin tones

Anyone have any insight on how this was achieved? Please don't say it's D&B because it's not.

Well, D & B can be an asset here as well as the Paint With Light II action at Action Central which I prefer. But, in this case, I went about it using color in various blend modes and curves adjustments for contrast and tone.
To start, I recorded the RGB skin values from your example, made a rough selection of Mozart's skin (borrowed from another forum) and painted with the RGB values in color mode. By making duplicates of this color layer and using various blend modes with each (i.e. soft light, linear light, etc), I got much closer. From there, curves adjustment layers for contrast and getting the RGB values matched worked best.

I would say this is an oil on the skin during shooting, contrast lighting and than D&B ...

I copied & pasted the example photo into CS4, selected the color sampler tool (eyedropper) and set it in the 'medium toned' area below the picture-left side of the nose. From the info panel set to RGB, I recorded R = 56, G = 28 and B = 17. I then used these values to set the foreground color and started painting and blending from there. If you watch the info panel, you will see that each blend mode has a direct effect on these values but each can be tuned separately with curves adjustment layers until you have the appearance that you are after.

I think you need good lighting and makeup for this, then I guess you can get the tone by playing a lot with channel mixer, shadows/highlights, last but not the least a fair amount of D&B...
