The Oleg Dou Effect (Revisited)

Hey people,

I posted here a while ago looking for some help on achieving this really surreal looking effect done by Russian art photographer Oleg Dou. He does these really bizarre looking porcelain doll like portraits and sometimes adds some extra manipulations to them. I was wondering if I could get some insight on how to really nail this effect. Here's my attempt at the effect and I was able to pull some stuff off but there are some things that I still need to work on and some pointers would be really awesome.

I would like to know how to make someone bald
I would like to know how to fully recreate the eyes. Its pretty evident that he rebuilds the eye lids and iris while removing stuff like the eye lashes (painting using paths and the pen tool?)

Any other thoughts, critiques, or ideas would be greatly appreciated!
